Need help fitting the date fields into two or more rows.

I have checked the form that you have created and see that you have used the Configurable List widget to display a Date Field and 2 Time fields.

The Configurable List widget can be formatted with the help of CSS codes that can be injected into the widget's Custom CSS tab — How-to-Inject-CSS-Codes-to-Widgets

Another thing that needs to be considered is the Form Width — The-Importance-of-Form-Widths

You can adjust the width of your form following that guide to fit the length of the Configurable List widget in your form.
