The text in one form field changes when it is delivered to my inbox

Hithere,

My form https://www.jotform.com/build/51186476551157 has fields named "Primary Phone Number" and "Secondary Phone Number" up near the top. When someone completes this form and it arrives in our inbox, both fields are listed as "Secondary Phone Number".  I have attached a snip of what it looks like when it arrives.  What could be the cause of this and how can we remedy it?
