Hi - the Thank you acknowledgment does not seem to be going out to anyone. They are left uncertain whether their report went through and also then have no record of when they did it.
any help appreciated

Are you referring to Autoresponder email, that is sent to the users once they submit the form?
Since when I checked your form, I didn't see any Autoresponder created:
Only two Notifiers.
So if you need information to be sent to the submitter, you have to create the Autoresponder: https://www.jotform.com/help/26-Setting-up-an-Autoresponder-Email
