Locked out of client forms

    Hi. I have been locked out of two of my forms stating that HIPAA is required. I am not a medical provider but a labor support doula. I don't need HIPAA. Please restore my forms for clients to fill out, thank you

    Hi DoulaNoleen,

    Thanks for reaching out to Jotform Support. I am sorry you are having an issue with this. If you are not a HIPAA-covered entity you will need to select that in the Data Page in your account. Let me show you how:

    1. Please log out of your account and clear your browser cache.
    2. Log back in to your account and click on Go Back on the HIPAA modal.

    3. Click on Profile Avatar in the top right corner.
    4. Select Settings in the dropdown.
    5. Select Data from the left panel, and you will see the HIPAA modal, click on No, I am not HIPAA covered entity.

    I think I've done all of that and it still has them blocked

    Thanks for your patience and understanding, we really appreciate that. I've checked your account and I can see that currently there are no forms locked due to HIPAA compliance. If you are referring to a different account. Can you please let us know the email address of it so we can check and see what's going on?

    Hi Noleen,

    Thanks for getting back to us. I checked your form and see that it has been disabled. It can be enabled easily in Form Builder. Let me show you how to do that:

    • In Form Builder, click on the Settings tab.
    • Next, under the Form Status, click on the Disabled option and change it to Enabled.
