I have checked the forms you have shared and analysed the data collected by the form and I could not quite capture which data violates our terms of use.

I would recommend if possible you could contact the user and request them not to use your email when sending his marketing emails.

Below is a link to our terms of use:

Could you please give us additional information about this.

A screenshot of the email content and the Sender would greatly help us tracing and confirming that the owner of the said form used it for spamming.

Please do note that we do not tolerate people using our service for such activity and if proven, we would immediately suspend the account and all the forms of the user. But we would need your cooperation so we can gather evidence and proof.

    Thank you for your cooperation.

It seems like you have no plans to solve this issue, the website is still
alive and I keep receive abusive spam emails
    from this person (attach).
I will just go further by opening cyber criminal investigation report on
the appropriate authorities in your country. You simply promote criminal
    activities over the internet.

The website is in no way associated with JotForm. We do not sell domain names.

The user was using a form that he/she created in JotForm as a contact form on that website. That website is selling email databases gathered from somewhere else and the JotForm form was used just to make contact with the seller.

The form has been disabled and the user has been suspended.
