I created a form, we are unable to access it

    Thanks for reaching out to Jotform Support. I’m sorry that you're having difficulties with this. As per the message shown in the screenshot, It seems our system detects that you're dealing with Personal Health Information. As required by laws within the US, everyone dealing with healthcare data should be under HIPAA. 

    For you to continue using the forms, then it is really recommended that you upgrade to HIPAA for additional safety. You can check this guide out for complete details on how you can upgrade.
