Show/Hide Condition: How to add a box for entering two mailing addresses?

I am creating a membership application that will ask for the member's business address, then a mailing address if different than the business address. Is there a way to add an option above the request for mailing address that allows them to click if the mailing adding is the same as business address, and then that would collapse the mailing address box? (a photo of a recent form that I submitted that has this option is attached - when I click that button, the address fields collapse and I moved to the next page.)

    Thank you.

    Hi Techforceonsite,

Thanks for reaching out to Jotform Support. This is possible by adding a Multiple Choice element and using a Show/Hide a field condition. Let me show you how:

    Adding the Multiple Choice element:

    • In Form Builder, click on the Add Form Element menu on the left side of the page.
    • Under the Basic tab, search for the Multiple Choice element and add it to your form. Show/Hide Condition: How to add a box for entering two mailing addresses? Image 1 Screenshot 80

    Configuring the Multiple Choice element:

    1. In Form Builder, click Multiple Choice element field.
    2. Then click on the Gear icon.Show/Hide Condition: How to add a box for entering two mailing addresses? Image 2 Screenshot 91
    3. Go to the General Tab.
    4. Under Field Label, add your preferred label. On my sample, I've added Mailing Address.Show/Hide Condition: How to add a box for entering two mailing addresses? Image 3 Screenshot 102
    5. Then go to the Options tab.
    6. Under Options, add your preferred name. On my sample, I've added same as business address.Show/Hide Condition: How to add a box for entering two mailing addresses? Image 4 Screenshot 113

Setting up the Show/Hide a field condition:

    1. In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
    2. Click on Conditions on the left of the page.
    3. Then select Show/Hide a field condition.Show/Hide Condition: How to add a box for entering two mailing addresses? Image 5 Screenshot 124
    4. Set the conditions like you see below and click on Save.Show/Hide Condition: How to add a box for entering two mailing addresses? Image 6 Screenshot 135

That's it, it should be done. If the form respondent click on the checkbox, the Mailing Address field will be hidden. Check out my screencast below to see my results:

We have a guide on how to clone a form here.

Give it a try and let us know if you need any help.
