Hello Kenneth,
Thanks for reaching out to Jotform Support. Sure, you can delete your account, but keep in mind that it will also delete all of your forms and data. Let me help you do it:
- On your My Forms page, click on your Profile Avatar at the top right corner of your screen.
- Then, click on Settings in the dropdown menu.
- On your Settings page, click on Account on the left side of the screen. Then, scroll down and click on Delete My Account.
- Now, in the pop-up warning window, click on Yes, Delete.
- After that, enter your account password and click on Verify. That's it.

Ronald, thanks for your prompt reply. However,
Clarification: I am not a business. I am just an individual who thought he was filling out forms to go to the doctor's office. I thought this was a free jotform account so I could go to the doctor.
I do not have an Avatar in the right-hand corner. There is a Circle in the right-hand corner. Under the Circle it states:

Admin Console
Settings
Logout
If I click on Settings all I get is a screen stating:
Account Type: FORM USER-HIPAA
Password
Name
Email
In the left-hand corner there is a rectangle with the word Account, but NOTHING opens up to scroll down to. There is no option to Delete My Account.
Can you please delete my account on your end? I am just an individual who has decided to not go to this doctor. I do not need this account. I thought I was just looking at forms.
