Fillable PDF: How to create one

    Help! I'm trying to turn the attached form into a fill-able pdf I can track once it is received. I'm having a hard time setting this up.

    Do you want to create a fillable PDF (to use offline) or did you want your respondents to apply online?

    May I suggest you attempt a fillable PDF, and see one before you make a decision. First of all the fillable PDF wont let you float entries beside each other. It doesn't look as good as a regular form.
