Hello!
I am a doctor, and my clinic is looking for a way to administer health questionnaires to clients.
For example, we want to create a candida questionnaire. Each question will carry a certain score. The higher they score, the higher the likelihood the patient has candida.
How can I use Jotform to create auto-grading questionnaires?

Hi Scott,
Thanks for your patience. You can use Dropdown or Single Choice, among a variety of elements to achieve this.Then, set Calculation Values to apply the appropriate score to each option. Let me show you how.
- On Form Builder, click on Add Form Element.
- Add your choice of element to your form, either Dropdown or Single Choice, for my example I used both.
- Select your Dropdown or Single Choice field, then click on the Gear icon for Properties.
- Click on Options.
- Toggle Calculation Values to On.
- Then apply a score to each of the options. For my example I set 1 to 10 points, 2 to 20, and so on.
After setting up the scores for your questions, you can add the Calculation Widget to your form and set it up to calculate the scores. Here's how to do it.
- On Form Builder, click on Add Form Element.
- Click on Widgets, then add Form Calculation widget to your form.
- Select the Form Calculation field, then click on the Wand icon.
- Set up the calculations by clicking on the Add Field, add your question, then click on the Addition Sign, add another field, and so on. Then, click Save.
Then, after setting this up, the Form Calculation field should generate a score based on the answers of your questions. Here's a screenshot of the result.
