Embedded form: Not showing

    On my Spring Start Up Repair Form the towns that are listed on the first page are not showing up in Internet Explorer so the form is useless.  I just sent out an email to 3000 people to schedule appointments and now they can't make them online. Please help right away.  Thank you.

    However, the embedded form is not http://www.krupskesprinklers.com/id144.html

    I checked it on Google Chrome and MS Edge. Both browser are refusing to load the embedded form.
