When I try to print from PDF, the dates are not showing but If I preview the application the dates are showing in PDF

I am not sure if I understand your query correctly. Do you mean to say that the date is not being displayed in the PDF submission report? As I tried to test the date field, it is displaying normally on the PDF submission report at my end. Could you provide us with more information on the issue that you are experiencing so that we can assist you accordingly? It would be helpful if you can provide us with the screenshot to take a look.

I have checked the form you shared with us and I can only assume that you are referring to the Start Date and End Date fields in your form. Also, if I'm understanding you correctly, the date fields are not appearing on the PDF copy of your submission when you download it.
It possible that the date fields are not included in the PDF copy of the submission because they were removed or were not added.
I would suggest that you customize the PDF copy of your submission.
Then, on the PDF Design page, click the Reset Button and save it after.
