Hello Irfandy,
Thanks for reaching out to Jotform Support and we’re sorry you’re having trouble logging in to your Jotform account. After checking this, we have found that your form has an offending field. The form has fields that collect usernames, and passwords, and use a copyrighted logo which, unfortunately, is against our Terms of Use.
For us to reactivate the account, we will have to delete the offending fields. Please note that doing this will also delete submission entries linked with that field.
Let us know if you agree with our terms so we can reinstate your account.
