The form does not prepopulate using the spreadsheet to form widget

Hi I am using a spreadsheet to prepopulate. I am only able to pull some of the fields. For some reason the other fields are not read but they are formatted the exact same way so i am not sure what i am doing wrong. 

I noticed that on your spreadsheet to form widget, what you have uploaded is a .doc file and not a .xls or .xlsx. The spreadsheet to form widget can only read spreadsheet document files.

If the problem persists after the necessary change, please let us know.

I updated it but it still wont read two columns. I am not sure why.

Just to make sure, the column titles in your spreadsheet should be the same as your field labels in your form's questions. You will also need to have a Unique Code column in the spreadsheet.
