Is there a way when I send out a survey or evaluation, to know who responded without asking for identifying information?

Hi Michigan_CASA,
Thanks for reaching out to Jotform Support. I’m sorry you're having difficulties with this. Although we don't have a way to determine who answered the form if there is no name field, we can still track who answered the form if we add a UTM tracking to the form and send it individually. Let me show you how to do it:
First, let's add a UTM field on your form:
1. In Form Builder, click on Add Form Element on the left side of the page, and drag, and drop Short Text widget on your form.
2. Click on the Gear icon of the Short Text widget, and the Advanced tab.
3. Scroll down, and click on Field Details panel.
4. Copy the Unique Name.5. Right click on your Short Text widget, and click on Hide.
Now, let's copy the form URL, and send it to the participant in a hyperlink:
1. Click on Publish in the orange navigation bar at the top of the screen, and click on Copy Link.
2. Compose an email for your participant using your preferred email provider's platform.
3. Add the form link in the hyperlink, and follow the pattern below:https://form.jotform.com/{formID}?{UniqueName}={NameOfParticipant}
Now, if the form has been submitted, you can track who submitted the form.
