How to upload PDF document as Smart PDF form

I want to use this file to serve as a fillable pdf. i can't figure out how to do it. Please advise.

Thanks for reaching out to Jotform Support. You can save your file as a PDF using adobe or any third party PDF editor. Then, open it in the Smart PDF builder. Let me show you how:

1. In My Forms page, click on Create Form.
2. Then click on Import Form.
3. In Import Form, click on Import PDF form.
   
   
4. Then upload your saved PDF document.
   

Give it a try and let us know if this works for you.
