Pre-Fill Jotform that is embedded on multiple pages

    I am building out a Custom Design Request form that I want to embed on multiple product pages of my shopify website. How can I get a product Name field to prepopulate based on the specific product page the form is accessed or submitted from. For example customer clicks the "Request Custom Design" button on a Baseball Jersey product page, which redirects the page to the section at the bottom of the page where the jotform is embedded, and the Product name is automatically populated with the name of that specific baseball jersey.

    Hi Frank,

    Thanks for reaching out to Jotform Support. You can only add one Product List element to each form. To my understanding, you'd like to redirect users to a specific form based on the button that they clicked. If my understanding is correct, you can create multiple forms and embed those forms to your website.

    If that’s not exactly what you’re looking for, can you explain a bit more so I can help you better?

    I want to embed the SAME form on all of my product pages.... Then have the product name field of the form pre-populate based on the specific product page it is being completed on.

    Hi Frank,

    Thanks for getting back to us. You can use the Get Referrer widget. This widget will retrieve the webpage URL where the form is embedded. Here's how to do it:

    1. In Form Builder, on the left side of the screen, click on Add Form Element.

    2. On the left panel, go to Widgets.

    3. Search for Get Referrer.

    4. Click/Drag to add Get Referrer into the form.
