Login Issues

    Hello,

    I am getting a login error that says that the login failed-suspended. I do not understand why my account would be suspended. If that could be explained and corrected that would be great!

    Your account is suspended because of the form - Wellness- Intake Form (Emind). Inside of that form you have fields that are used collect Credit Card information and that is against JotForm Terms of use.

    Phishing - You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

    Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

    In order to activate your account, it is required to agree to our terms. We will also have to delete mentioned fields from your form. 

    If you want to have a payment field inside of a form, you have to use one of the JotForm payment tools.

    Same issue here?!?! Seriously, i have zero field asking for any private stuff. Come on!!

    I deleted the credit card fields form your form and I activated your account. Please do not create similar forms. If you want to have a payment field inside of a form, you have to use one of the JotForm payment tools.

    I am getting the same message. I have changed my password twice, but my developer is waiting for this! Ugh.
