Form is locked due to HIPAA

    Hi we are NOT a HIPAA compliant organization, please unlock the Patient Release form ASAP.

    Thanks for reaching out to Jotform Support. I'm sorry you're having trouble with this. Have you gotten an email about HIPAA compliance? The email is intended to confirm whether you or your business are a HIPAA-covered entity. If you have not yet responded to the email, please open the first HIPAA-related email you got and click the orange "Go to My Account" button, as shown below.Need access to our own form that does not have PHI on it Image 1 Screenshot 30 Screenshot 10

    Then, click the "NO, I'm not a HIPAA-covered entity" button to keep your forms from being disabled:Need access to our own form that does not have PHI on it Image 2 Screenshot 41 Screenshot 21

    If you have clicked No, Jotform will not ask your account to comply with HIPAA again.

    Hi, yes I got the email, and clicked "No" on the question several times
    this am, form is still locked. Please advise.

    Thanks for patiently waiting. I received an update from the Relevant Team. The form is already activated, according to the Relevant Team. However, it seems like it's been disabled through the form settings. You can re-enable it on the form settings. You can check this guide for more details on How to Enable or Disable a Form.

    Give it a try, and let us know how it goes.

    Hi,

    Enabling the form worked, and I'm very dissatisfied with how long it took to get this simple resolution to the problem.

    I created a duplicate of this form days ago as a temporary measure and now have to revert the form url.

    Just making it clear this took way to long to resolve.

    Hi - we are having the same issue. When I clicked the link in the email, the only option was to pay for HIPAA compliance. We are not that kind of organization and it is not required. Is the plan here to get people to pay for it in order to unlock forms because jotform is so slow? We do not collect PHI, nor have we ever. We are a health care related organization and use a lot of words that I see why this was triggered by your AI - but no, we do not need HIPAA coverage.

    Edited to add - I am a new customer and we pay a lot of money for form websites. I was considering consolidating all of our business to Jotform, but i'm not impressed by this and will reconsider.
