Additional Form from successful Referral

hello, I referred CHEFGIRL to sign up and they are verified. How do I add the free additional form?

Please note that for Starter accounts, you are only allowed to have 5 active forms. In order to add more form count, you send more referrals, or you can upgrade your account.
