Why is our form not submitting?

criei o formulario mais nao sai da tela para enviar 

fica dizendo para aguarda e nao sai dessa tela

I tested your form and i was able to make a submission as shown below.

What I have however noticed is that your form is redirecting to the same form upon submit. This may be confusing to user and will give a feeling the form did not submit.
