I am unable to choose my forms.

Could you be more specific on the issue you are experiencing, so that we can pinpoint the problem better.
When I log in to your account I observe no issues regarding your forms. Also it would be nice to provide some screenshots of the problem if you can.

When I click on the form it does nothing. In the past I could select the form and choose to edit, view or see submissions etc. Now nothing happens when I click on the form and there is no box on the left of each form to select or work with it. I cannot create a new form or do anything on the My Forms page.

I can not replicate the issue on your account. I was using Google Chrome, the same as you.
Please try to select your forms using incognito mode. If you can select the forms there, clear your browser cache. You can also try to use some other browser.
If the issue still persists on your end, provide us a screenshot with a console report.
