The emails that are sent out by the auto responder send all the data to the users but the internal email is sometimes missing elements. I have attached examples below. Notice the blank line in the Notification email.

I have checked your form and your email notifications. It seems that the first screenshot you have given is from the notification email while the bottom two are from the autoresponder.

You can however, edit the email contents of both notifications.
