can't get the create form Botton to work

How do I get the create form Botton to create the form I got help making

Can you try to Clear the Cache and Cookies in Your Web Browser and try it again? If the same thing happens again, Can you take a screenshot of what’s happening and send it to us? This guide will show you how to do that.
