Notification Email: Form field is showing as Type a Question

    Hello,


    My form titled 'Listing Intake Form' looks fine when I view it in my account and it looks fine when I look at it from the link I send. However, the second question in the form is populating the same as the first question when I get the form back. Any help with troubleshooting on this?

    My first question is 'Agent's Name'

    Second question is 'List the address with City of the property'

    When I get the form back the second question say's 'Type a question' and the answer is populated with the agent's name.


    So frustrating to set up forms and then have to go back and ask questions that are already meant to be on the form. Please help!

    Hello Tiffany,

    Thanks for reaching out to Jotform Support. I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. It shows the First field and the second field in your form does not overlap. Check out the screencast below to see my results:

    Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.

    Hello, thank you for sending. As I stated previously it’s correct when I open it as well. But when the form comes back to me it does not put the address in the blank, the email I get has the agents name in the first question and again as the answer in the second question. I can’t figure out how to fix that because it looks correct when I open it.

    This is what the form looks like when it's completed and comes back to me. Instead of having 'address with city' it says 'type a question' and instead of the answer, it duplicates the agents name again

    Hi Tiffany,

    We are sorry you are having difficulties with this. I made a test and was able to replicate the issue. To resolve it, you need to delete your current notification email, then create a new one:

    • In Form Builder, in the orange navigation bar at the top of the page, click on Settings.
    • In the menu on the left side of the page, click on Emails.
    • On the right side of the Notification field, click on the Trash Can icon, and delete the existing Notification Email.

    Now that we've removed the old Notification Email, we'll just add a new one. Let's do it: 

    • While you're still under the Emails tab on the left side of the page in Form Builder, click on Add Email.

    • Next, select Notification Email. Do not forget to click the Save button.
