Hello,
I would like to download a PDF version of a Performance Evaluation Form that I have customized in JotForm. When I attempt to download the form, the date is pre-populating with today's date and I don't want a date to pre-populate at all. How can I change the pre-populated date settings?
Also, when I go into the "Open PDF Editor" option in JotForm to download the PDF version of the form a message appears at the top of the form that says "In order to submit this form, you should open it in Adobe Acrobat Reader". How can I remove this message from showing on the downloaded PDF version of this form?
Here is the link to the form: https://form.jotform.com/240664377404155I've also attached a screenshot of the downloaded form for you to view.

Hi Gianni,
Thanks for reaching out to Jotform Support. I’m sorry you're having difficulties with this. To download the PDF version of your form, you need to open it in your PDF editor. Let me show you how to do it:
1. In Form Builder, click on the Downward Arrow icon on the upper left part of the page, and click on PDF Editor.
2. Click on Download on the upper right part of the page.
As for your other questions, I've moved those to a new thread. You can check those out below:
- How to download the form with empty date field?
- How to remove the yellow banner at the top of the screen when opening a PDF form?
Let us know if there’s anything else we can help you with.
