Power Automate: Failed to retrieve dynamic outputs error

I tried to trigger Power Automate when a response is submitted but got the following error:

I only added a trigger (When a response is submitted) to the flow. There is nothing else

Thanks for getting back to us. I checked your account but I don't see any form in it. Can you share the URL of the form in question? Also, for us to test the issue on our Power Automate account, can you provide more details on the steps you took to create the flow? For this first step, may I know that did you choose among the options in the screenshot below?
