I have 2 forms Nursing CAP Intent Submission and Nursing CAP Documentation Submission Form. How can I link the 2 forms together so that information on Nursing CAP Intent is forwarded to Nursing CAP documentation submission form?

You can use URL parameters to prepopulate the value from your first form and pass it to your second form.

Thank you. This looks complex. Can you help me with this? here is the url https://form.jotform.com/adunbar2/nursingcapintentform2 I would like to place fields that is on the CAP intent (Name, employee number, CNMC email, applying for Level 1, applying for Level 2, applying for Level 3) to the CAP Documentation Submission form. Those fields are not currently available or one to one match in Nursing CAP Documentation Submission form

I have checked the two form, we need to match the fields from form 1 to form to be able to populate them when they are submitted.
You have a name in your nursing CAP but you don't have it in your CAP intent. Also, we need to match the Question Text of both forms. If its employee ID# the second form should be the same.
Here is a sample URL parameter that would transfer the value of field name to the next form.

I have inserted the URL parameter code to the link in the Thank you page, or you can paste in the Redirect to external link.

I have added the fields in the Intent form so they now have the fields (Name, CNMC Employee Number, CNMC email and I am applying for CAP Level). These fields are also available on the CAP Documentation submission form.

Can you give me a url link that would copy the data elements from the above mentioned to the CAP documentation submission form?

You can try this URL parameter code to populate your CAP Documentation Submission.
You can add this code in your Thank you page> Redirect to external link so that when your form is submitted it will directly open the link to the other form.

thank you so much the only thing that did not work was the "I am applying for CAP Level" field. This field did not carry over to the other form. Can you send me a new updated link?

Sorry for bothering you again but I have modified my forms because there are issues with the field (I am applying for CAP level). Can you modify the link to remove the I am applying for CAP level field and replace it with (Applying for CAP Level 1, Applying for CAP level 2, Applying for CAP level 3)? Thank you

I have checked your form and the modification you have made. To trigger the URL parameter code, we need to add a a Change "Thank you" Page condition, to change the URL parameter depending on the Level of the user.
First, we change the URL parameter of level 1 by selecting the fields that would select Applying Cap Level 1.
Now we use the rule "ANY" so that if you choose any of the possible answers it will still redirect to Cap level 1. Then we use the Redirect URL after submission, to redirect to the next form.
URL paramater for Cap level 1:https://form.jotform.com/80516314944153?session={email}&name[first]={name:first}&name[last]={name:last}&cnmcEmployee={id}&cnmcEmail3={email}&typeA228={applyingFor}
Do this steps again for Cap level 2 and three and use this URL parameter code.

Thank you. The link for Cap level 1 and level 3 works but it seems something wrong with the CAP level 2? Can you resend me a new link that defaults Applying for cap level 2?

I'm sorry, but could you please clarify what exactly is not working for CAP level 2 option? I tested my colleague's form several times, but I wasn't able to determine.

On the intent form when the Applying for CAP level 2 is Yes on the next form Documentation Submission form it should also default to Yes but it did not see screenshot

Also it looks like Applying for CAP level 1 is behaving the same way it is not defaulting. Only Applying for CAP level 3 is defaulting.

It still did not work. See below screenshots and now Level 3 default is also not working. Are there issues with default with single select option?

I also recommend using the JotForm Prepopulate Form Field app to generate the form URL with prefilled values.
1. Make sure you were logged in to your account then use this link : https://prepopulate.jotform.io/
2. Select the form you want to be prefilled.
3. The URL will be created which you can copy.
4. Paste the copied URL into your source form redirect URL conditional statement.

I believe my urls are correct but I am not sure why the single select options are not defaulting. Highlighted area above is the fields for Applying for Level 2. One field is on the Intent form and the other field is from the Documentation submission form. I want the fields from Intent form to go to the documentation submission form

I apologize for limited details from my previous response. I was actually doing further test as well the reason for my delayed update.
I used the Prepopulate app to better test the correct URL output and identify the field names needed.
And I found that Single option field was indeed not being populated when using the prefilled URL method.

I could not find any other way to prefill the Single option field.
I think it is a bug in the Single option field. I have submitted a bug ticket report to the next level support about it so they can further investigate.
I will create a separate thread for the bug ticket report. I will update you here shortly.

ok thanks I have changed my fields to text entry. I am now using this URL for Cap level 1 but still not working can you check if I have this correct?

I noticed that Level 1 and Level 2 are being changed to Level 3 after the form loads completely. This seems to have something to do with the session parameter.
If you remove the session parameter, the correct Level will be filled.

Is there any particular reason for you to use the session for those URLs? We received an input from our colleague, that session data will override the URL data.
So it would be better if you could use the prepopulate URLs without session.
Is that possible for your workflow?
