My forms have been marked as disabled due to needing HIPPA upgrade. I am making these forms at the request of a business. I am not a covered entity. Could you please enable my forms so that I can continue to make changes as needed. Thank you.

    Hi Erica,

Thanks for reaching out to Jotform Support. I’m really sorry that this is happening. I can help you with the solution. If you are not a HIPAA-covered entity, please follow the steps below:

    1. In My Forms page, click your Avatar in the upper right corner of the page.
    2. Now, in the dropdown, click on Settings.
    3. Next, click on Data on the left panel.

Forms got disabled due to HIPAA Screenshot 20 Screenshot 104. Finally, select NO, I'm not a HIPAA-covered entity in the modal.HIPAA: I am getting messages that I need to upgrade my plan Image 2 Screenshot 41 Screenshot 21 Screenshot 31 Screenshot 31 Screenshot 21 Screenshot 31 Screenshot 21

If you do not see the modal, please log out of your Jotform account and log back in. Once you're logged in, you will see the pop-up modal. Just click on "NO, I'm not a HIPAA-covered entity". Keep in mind that this process may take up to 30 minutes to a few hours to reactivate the forms.

If the form is still disabled after an hour, try to Enable the form. We have this guide on how to do that.

Give it a try and let us know if you have any other questions.
