Sorry to hear about your unavailable forms.
To get it fixed as soon as possible, I would highly advise you to provide more details of the issue on this thread because customer support isn´t available via Phone.
By doing that, I´m sure the technical support team will advise you on the best course of action.

We’ll be happy to help.Now, coming back to your question. I checked your forms and it looks like you are referring to this form which is currently unavailable. When I checked the status it was set as Disabled. You will need to Enable the form to use it again. Let me show you how:
- In Form Builder, click on Settings.
- In the Left Pane, go to the Form Settings.
- Click on Disabled under Form Status and choose Enabled.
- Then click on the Save button.
