Issue with locating form linked to the QR code

Hello, I used JotForm to create a quote form, and published the QR codes on my business cards. The form went away, and now I have over 2000 business cards with the QR. Is there anyway to locate that form (not in my archieve), or can I send you the QR code to input into my new quote form?

Thanks for reaching out to Jotform Support. Unfortunately, publish QR code is not editable. But, we can check the QR code to find your form. Can you take a screenshot of it and send it to us? This guide will show you how to do that.

Thanks for getting back to us. Unfortunately, the QR code on your image is too small and a bit blurry and cannot be recognized by the QR App reader. Can you please try sending a bigger and clearer screenshot of the QR code? Also, can you verify the title of the form? Can you check if this is the form you are referring to?
