Forms unavailable - Asking for Hipaa upgrade

    None of my jotforms are working. It's saying they're unavailable. What can I do?

    Hi Connor,

    Thanks for reaching out to Jotform Support. Reason why your forms were unavailable is that our system detected that your Form/s is collecting PHIs/Health information from individuals. If your forms do not require HIPAA, you can disable it under Account Setting. Let me show you how:

    • On your My Forms page, click on your Avatar/Profile Image on the top-right side of the page, and then click on Settings.
    • Go to Data in the menu on the left, and select NO I'm not a HIPAA-covered entity in the pop-up modal that appears on the screen.

    If you are not a HIPAA Covered Entity, and your forms are collecting PHIs/Health information, you will need to upgrade your plan to Gold Subscription so you can make your account HIPAA Compliance. You can check this guide on How to Upgrade to HIPAA Compliance.
