Help prepopulating information from one form or table into a form

I have tried using both information submitted on a form and information from a table to prepopulate information on a new form but it doesn't seem to be working. Could you help me figure out what I'm doing wrong, please?

My first form is 2025 Participation Form. The second form is Additional 2025 Participation Form.

Any help would be greatly appreciated!

    Hi SCAC,

Thanks for reaching out to Jotform Support. I’ll need a bit of time to look into this. I’ll get back to you as soon as I can.

    Hi SCAC,

Thanks for your patience. You can set a prefill in Publish using a submitted form or tables, in Form Builder. Let me show you how.

    1. On Form Builder, click on Publish on top, then click on Prefill on the left side.
    2. Click on Tables Prefill, select a submission table data.
    3. For this part, please be reminded to match the fields accordingly. Then, click on Save Settings.

    4. After saving, click on Add A New Prefill.
    5. Select the submissions you want to add, then click on Create.
    6. Select prefills you want to send, then click Send Invitation.
    7. Fill out the email, then click Send Invitation.

When the email is received and a user clicks on the link, the form will be prefilled with information.
