Jotform: How to change page title of form?

Hello I used A template Off jottform I was wondering if i could possibly Remove the perosn name off the jott form ? They name show up when i Sent the intake to my clients

Does your form shows a different title on the browser tab? You can actually update the page title.

Here, let me show you.

1. Go to your Form Builder and click on Settings on the middle top.
2. Then Select More Options.
   
3. Go to Page Title below and enter your new title.

Thanks for reaching out to Jotform Support. If I understood you correctly, you want to change the page title that shows on top of the browser when someone open your form. Sure, we can change the page title easily in Jotform. Let me show you how:

• In Form builder, select Settings at the top.
• Click on Show more options.
• Scroll down to Page Title section & edit the Title to what you want.
