Social security number, driver license number bank account and routing number

    I own a small personal loan company. We need to ask the applicant to provide their social security number, drivers license number, bank account number and routing number for pre-approval of their loan.  How do I keep these features on my application yet still comply with your terms and conditions? I do not have a terms and conditions page nor a signature page. I need to keep my application up and running without any interruption.

    Helloy Amy,

    SSN and Driver's License Number are considered sensitive information. When you ask these information to your form, you must also add the Signature and the Terms widget in order to comply with our policy. This is required in order for the users to agree to your terms and sign them when they provide such information.

    About the bank information, unfortunately, we do not allow asking these on forms even if used for legal purposes. We highly recommend using the payment integrations like Paypal, Stripe, Authorize.net and etc. You can find the guides here https://www.jotform.com/help/chapter-7-Payment+Forms.
