Why isn't my form updating with my changes?

Hi,

I'm trying to create a basic configurable list with a drop down menu and a couple of text fields. When I create it in the editor it looks fine but when I either preview it or go to the published url it displays the "default" widget info. What am I doing wrong? I have attached screen shots
