For Those having issues adding Jotform to Facebook

Go into settings, Edit Page, Contact Us, Edit Settings, Change Tab Name, Save and Okay. I did that now I have the Contact Me tab with my form..

I do not have an option to add a contact us form. This is still possibly limited to only those pages with 2k+ likes.
