Added a form to my website - cannot get to submit

I added a form to my website - when you complete the form and click submit it just says "Please Wait" and the form submission never goes through...just keeps saying "please wait"  I have tried deleting the link and adding a new link and also embedding the link - neither of these have solved the problem.

I was able to replicate the issue when I did a test submission in your form. Here's a screenshot:

Please try to re-embed the form using the iFrame method. This will prevent script conflicts between the form and the webpage. Here's a guide: https://www.jotform.com/help/148-Getting-the-Form-iFrame-Code.
