Account disabled due to my credit card form, I can make the change to make it fir guidelines

Hi Sway,
Thanks for reaching out to Jotform Support. I checked your account and can see that it is currently suspended. The form with the ID 240674235816156 is collecting Credit Card Information.
Take note that collecting Credit Card Information using regular text fields is against Jotform's Terms of Use. Due to our PCI certification, we are required to deny the collection of credit card details on regular forms.
However, we do have other PCI-compliant ways to do this. You can use one of our Payment Processor Integrations and integrate them into your forms then use the "Payment Authorization" feature.
Using this feature will allow you to collect your customer's payment details and charge them at a later date. But you still can't directly collect credit card details. You can check out this guide on How to Enable Payment Authorization.
Since the form doesn't have any real submissions on it, I have gone ahead and deleted the offending fields. I also reactivated your account, please reset your account and try logging into it. Let me show you how:
1. Go to the Jotform Login page and click the Forgot Password.
2. Enter the email address of your account then click the Send Reset Instructions button.
3. Check your email account for the password email reset then click the Reset Password button.
4. Enter your account's new password then click the Create a New Password button. You can check out this guide on How to Reset Your Jotform Password.

Thanks for getting back to us and we’re sorry you’re having trouble with this. Your account has been suspended due to your Card authorization Form breaking our Terms of Use of collecting credit card information in plain text fields.
Due to our PCI certification, we are required to deny the collection of credit card details on regular forms. However, we have other PCI-compliant ways to do this. All you need to do is choose any of the payment processors available on our platform to integrate into your forms. You still won't be able to collect credit card details directly though. If you only want to collect your customer's payment details and charge them at a later date, you can take advantage of a feature called "Payment Authorization". To learn more, kindly follow this guide on How to Enable Payment Authorization.
If you agree to follow our Terms of Use and allow us to delete the offending fields mentioned above, we can go ahead and lift the suspension on your account.
