Conditional formatting within Appointment Scheduling

    Hi!

    Is it possible to use conditions within an appointment booking form?

    I'd like the first question to ask if the booker is a new client or returning client, and if they are new, I would like the form to include intake questions. I don't want returning client to have to fill out these questions every time.

    If this is not possible within the appointment booking, is there a way for that first question's answer to trigger the sending of another separate form (a new client intake form)?

    Lastly: is it possible for the answers to the new client intake section or form to trigger the creation of a new client file from a template, populating specific fields with the data gathered?

    Hello Audie,

    Thanks for reaching out to Jotform Support. Yes, it is possible to show the question based on the answer of the user. We do have a complete guide on How to Show or Hide Fields Conditionally that you can check out.

    In your scenario, we can show the intake section only when a new client is filling out the form. This is how it will work :

    Let me walk you through how you could create that form :

    First, you need to create the whole form, including all the questions for existing clients and the new client section.

    Second, you need to add a condition to show the intake section when user selects new user. This is how the condition will look :

    1. In the orange navigation bar at the top of the screen, click on Settings.
    2. In the list that appears, click on Conditions.
    3. Select the Show/Hide FIELD option, and create a below condition.
    4. Create the condition shown in the below screenshot
    5. Click on Save Button.
