    Hi Michael,

Thanks for reaching out to Jotform Support. We’re sorry you’re having trouble logging into your Jotform account and accessing forms. I confirmed that your account was suspended due to the Jotform system detecting fields or words on your form that violates our terms of use. Please have a quick view of Jotform's Terms and Conditions to know more about the restrictions and to avoid this happening again.

Due to our PCI certification, we cannot allow the collection of credit card details on regular forms. However, we offer other PCI-compliant methods for this purpose. Simply select one of the payment processors available on our platform to integrate into your forms. If you need to collect your customer's payment details to charge them later, you can use a feature called “Payment Authorization”.

I have deleted the offending fields and activated your account again. Your form is back online and should work as expected. Do try to sign in to your Jotform account and get back to us if you have any trouble.
