We have been having issues receiving emails once the forms have been submitted. I have verified that the recipient emails are correct. I also filled it out myself as a patient and I never received an email confirmation. Please help!

We apologize for the inconvenience.
Since you are using a custom domain, please try setting up your SMTP as your sender address and see if that works.
