    We are having trouble with our jotform patient registration form. In the middle of the form it will shoot a error message saying "a problem occurred with this webpage".

    Apologies on any inconvenience. I have tested the form you shared, but was able to fill it without errors on my end:

    Could it be a temporary issue or you are still experiencing it? 

    Would it be possible for you to make a screenshot of the error, so we could check it closer?
