So I keep getting this error on my forms, I get an initial reply and then there has never once been a follow up.
Why do you even have a support team?
Who keeps track of the support inquiries?

Always the pdf attached is different than the results on the form.
In addition, blank fields are still showing after selection empty fields not to show up.
Can you state of the art support super nova team handle an inquiry like this?

As to your questions:

My apologies, can you please explain what you mean by 'different'? Does pdf file include incorrect information or it has unexpected styling?
In addition, blank fields are still showing after selection empty fields not to show up.
I have checked your form and can see that you have customized email template table for notification emails. The default table is connected to form fields to provide the features to "Update Email" and "Hide Empty Fields". But, if it is broken, this options can not work properly.
If you want to get this features back, I can suggest to reset email template to default by deleting current Notification and setting up new one: Setting-up-Email-Notifications
