Missing info when printed out 2

Attached is another screen shot. It would not let me upload 2 different screen shots.

Some information previously on our forms are now missing for no reason. My boss has put them back. You can clearly see that they are on there, but when the client submits them and I print them out, that information is not there. Attached are the screen shots. Please help!!
