My Form disappears in the mobile version when you scroll and a portion never shows

my "How can we Help you" Form disappears in the mobile version when you scroll and a portion never shows. 

I cannot replicate this problem on both my Android and IOS devices. Were you able to fix this? If not, what is your mobile brand and model?

As you could see, the form does not disappear there. I scrolled down and back to top, it is there without any issues. 

I'm using the Iphone X IOS 12.  It appears at first then disappears as I scroll. 

I have tested your web page using my iPhone 7 and replicated the issue.

When you scroll the form seems to be not visible, but when you  stop scrolling the form would be seen again.

Can you try to embed your form using the Iframe code and let us know if the issue still remains?

I tried that already. NO changes. 

This seems to be a case of a script conflict with WordPress. Many others are using the same embed format, and they don't have issues with it. 

Would you mind putting the iframe codes again? We need to troubleshoot the problem with that embed method. 

Also, we need to know if you are using a module or block or embed in WordPress to place the form codes. Can you send us a screenshot? Can you try adding the embed codes on the default text block? 

I put the iframe into the default block like you requested. It doesn't space out as good as the short code.

Can you please provide us a screenshot of the output when you used Iframe to embed your form using the default block?
