Hello, I'm having a bit of difficulty with some of my forms. I'll post a link below to one I am working on right now below. But my problem is, a lot of the information that is on the Form itself is all out for display. When it should be hidden.

Users who fill out the top half of the form should not be able to see the bottom half or fill any of the information in those boxes. Anything below the "Submit to Supervisor" button should not be viewable to them at all.

And from there, once the supervisor opens the link to view the form. They should be able to see the information that was hidden to fill it out and tell the user if their request has been approved or not.

I am sorry for the trouble caused to you. I did check your form and found that you have added several conditions to show / hide form fields. It seems some condition conflict is causing the form fields to be displayed.

Please note that there is a better option to show / hide multiple questions by taking advantage of form collapse fields. You may like to take a look at the following guide which should help you:  https://www.jotform.com/help/297-Show-or-Hide-Multiple-Fields-at-Once-Using-the-Section-Collapse-Field

I have cloned your form and disabled all the show / hide conditions you have added and added one condition to show only the first set of question you want to display to users. Please take a look at the following cloned form and see if it works as expected:   https://www.jotform.com/82665296196976

I would suggest you to please explain your other workflow requirement on how you want to display the Supervisor or HR approval process and we will help you add conditions to achieve your requirement.

We will wait for your response.

Our work flow goes like this:

User/ Requestor opens the link to the form, they input the necessary information such as their name their supervisors name, the date and what type of request they are putting in and what days they are requesting off. The type of request they select, reveals the proper section of the form for them to fill while leaving the other hidden. They will then fill in the information in that section  Once they have filled out that information, they should then submit the form.

The form should then, depending on who they choose as their supervisor. Be forwarded to the supervisor for review. The supervisor should have the ability to approve or disapprove the request and provide reason as to why they disapproved the request. If they select approve the form should head on to our HR person for review. If it is disapproved it is sent back to the requester who filled out the form telling them it was disapproved and why.

When/ if the request reaches our HR person, they should have the ability to approve/ disapprove the form. With them giving a reason as to why they disapproved the form. When HR approves/ disapproves the form, it should then be sent back to the requester telling them that their request has been approved or disapproved and why it was disapproved if applicable.

We use a third-party system (Zapier) to update our calendars to make sure we record who has requested time off and when. But I am not worried about that right now. Just need to get this part up and running again.

    Thank you for your assistance.

First thing, you just need to configure to SHOW fields based on the selection. If all "requests" has corresponding fields, then configuring SHOW condition will automatically hide the fields as default then will only show up when proper selection is made.

As for the approval, I wonder if you have already checked the guide below.

    https://www.jotform.com/help/432-A-General-Approach-on-Building-an-Approval-Process-Workflow

Please check and let us know if you find any difficulties following steps. There is a demo form in the guide that you can clone and examine.
