How to embed a form on a web site?

I have an intake form I have made and cant link it to my website. I have always just gone to the form made, click on it show it shows like this https://form.jotform.com/80455261438154 and link it on my site. When I am on my website the link does not work. It works for another intake form so I am not sure why this doesnt work. 

I cant also find another jot form I made that use to work as a link on my site.
