PDF copy of submission missing questions

the email lists these questions (list of checkboxes that all need to be checked off), it's also being synced to a google sheet and they show up there, but for some reason those questions aren't making it to the PDF. Even if I choose the result from the View Submissions link and then Download a PDF, those questions are skipped. It seems to be happening with the hidden fields that get revealed when choosing a Type of Program. This works fine on our other form: https://form.jotform.com/80874793402160 ... I can't see a difference in any settings.

Try either updating the PDF report to ensure all fields are included:

Or resetting the layout all together.
