Hello.
I saw some information online indicating auto suspension of accounts. If this is the case, I can't use this product because I can't keep worrying if it is still enabled.

I cloned the form and it still is not working. Please help and tell me what to do to prevent this in the future. -

Your account is active. But you've set a date limit in the form, so likely that's causing it previously. You still have it there, set to November 1st. Please note that when November 1st comes, your form will be automatically disabled.
